In the first stage of this program,
we have participants who have completed the initial
criteria for application into
the program. There are many benefits
for participants who have been selected through a competetive process.
Participants for the first cohort
have been selected and notified.
The following steps are now
necessary for participants to continue:
1. Apply
Online
to the Arizona State University Graduate School. Your application is
for MEd Educational Administration & Supervision, Tempe Campus.
Use the code ED540827 when applying online.
2. Send official transcripts
for all courses taken up to now to:
Arizona State University
Division of Graduate Studies
Admissions Office
P.O. Box 871003
Tempe, AZ 85287-1003
The participant is responsible
for contacting all institutions of higher education in which she/he
has attended, requesting official transcripts to be sent to the
address above, and paying the transcript fee.
3. The grant will pay the $50
application fee. Indicate "check" online and then notify
Ms. Emerson when you have completed the online application and she
will then process the payment.
4. Complete the ASU
Extended Education Registration form for the Spring
2006 courses. Completed the areas highlighted with a line of blue
checkerboard only. Submit the form to Ms. Emerson or Ms. Nakaidinae.
5. Complete the Arizona
Residency form. As a Navajo participant your courses
will be considered "in-state." Submit the form to Ms.
Emerson or Ms. Nakaidinae.
6. Read about the ASU
Immunization Requirements and
complete the Measles forms.
Submit the form to Ms. Emerson or Ms. Nakaidinae.
7. Read and complete the Payback
Agreement form.
Submit the form signed and notarized to Ms. Emerson or Ms. Nakaidinae.
8. Complete the Internship
Release-Time form. Submit the form to Ms. Emerson or
Ms. Nakaidinae.
9. Read and ask questions about the Program
of Study form. This form will be updated each
semester as courses are completed and grades are assigned.
